Dimension 1 / Evidence trail

An inspector asks for the full timeline of one incident from last quarter. How long until it is in front of them?

Minutes. One system holds the governed timeline. Same day, if the right person is on shift. Days. We stitch exports, logs and inboxes. Honestly, we would reconstruct it from memory.

Can you show where a number in a compliance report came from, back to the raw record?

Yes, every figure links to its source record. Mostly, with some manual digging. The conclusions exist, the trail does not. We would have to ask whoever built the spreadsheet.

If your most experienced ops person left tomorrow, how much operational history walks out with them?

Little. Decisions and context are recorded as they happen. Some. Key events are logged, the reasoning is not. A lot. They are the memory for their area. The operation would partially forget itself.

Dimension 2 / Incident memory

Near-misses and small deviations (a level-sensor drift, a postponed ticket) are:

Known instantly. Assets and commitments are linked. Someone can work it out in an afternoon. Commercial and ops would need a meeting. We would find out from the customer.

Contract renewal windows are watched by:

The system. Alerts fire months ahead, tied to utilization. A shared calendar or spreadsheet someone maintains. The person who negotiated the deal. Renewals have surprised us before.

Do you know your counterparty concentration (largest customer as share of throughput) right now?

Yes, it is a live number with a threshold alert. Roughly, from the last quarterly review. Finance could calculate it if asked. Not without real work.

Dimension 4 / Audit & regulatory readiness

Audit preparation at your site takes:
